TDOH Grant Employment Opportunities:
South Cumberland Community Fund has been awarded a $2 million grant from the Tennessee Department of Health in the department’s Healthcare Resiliency Program. Twenty organizations working in rural areas across the state were selected to conduct programs in their communities. The Fund’s project involves collaborative partnerships with Southeast Tennessee Human Resources Agency, Mosaic Recovery Center, GRACE, Beersheba Springs Medical Clinic, and South Cumberland Community Clinic. Funds from the grant will support the development of fixed rides, van rides to medical appointments, a software-supported volunteer-provided ride service, and support for individuals in recovery who need transportation to medical and legal services.
All salaried positions come with a QSEHRA (health reimbursement account), and a Simple IRA. Employment is through the duration of the grant, which ends in 2028. The grant accountant role will be held by an independent contractor.
Program Manager
The Program Manager will oversee the planning, execution, and monitoring of grant activities that strengthen mobility network infrastructure, expand access to services, and support resource distribution through community partnerships. The Program Manager will report to the Executive Director and work closely with internal teams, external stakeholders, and community partners to ensure regulatory compliance, track progress, and integrate community feedback. Grant Accountant
The project accountant is the post-award financial manager for a $2 million grant from the Tennessee Department of Health. The project accountant will work with contracted agencies to ensure that expenditures are made in a way that can be reimbursed through the grant, following all legal requirements and collecting all required documentation. The project accountant will also prepare accurate reimbursement reports for the Tennessee Department of Health and ensure that reimbursements are made, following up with TDH personnel who may have questions about reimbursement requests and documentation.
Experience in the post-award management of federal or state grants is a must. Thorough knowledge of federal and state purchasing regulations as described in the Procurement Procedures Manual for Tennessee. The candidate must also possess interpersonal skills and the ability to work with partners with a wide range of experience in grant management and requirements.

Rideshare Manager
The Rideshare Network manager will take the lead on recruiting volunteer providers of rides and ensure that they are trained, prepared, and set up for reimbursement through the Assisted Rides software. The Rideshare Manager will work closely with the mobility network coordinator hired by our collaborator, SETHRA, and together will be the public face of the program with both drivers and people needing rides. The Manager will monitor ride requests from Assisted Rides as well as the SETHRA ride reservation system, and together with the mobility network coordinator will dispatch services to clients efficiently. Community Health Worker Coordinator
Working with the clinics and collaborative partners who will be hiring Community Health Workers, the CHW coordinator will lead recruiting and training a group of individuals who can serve as trusted community members who accompany patients through their health care journey. The Coordinator will take care of all arrangements for helping new CHWs successfully complete certification, will train CHWs on intake and the use of data to gain greater efficiencies in health care delivery across the service area of the grant. The Coordinator will be the lead CHW, coordinating work across Grundy County to enhance patient outcomes for vulnerable populations.
A successful applicant will either need to be a certified Community Health Worker or seek CHW certification and will take the lead on arranging for certification for the entire cohort of CHWs.
